There are a few simple steps to nominating your All Star. The first step, filling out the electronic form, is required. The last two steps are not required, but would greatly help us with the publicity surrounding the Monthly Winners. If you have any questions please contact us at triangle@advocacyla.org

• Fill out the All Star Nomination Form.


• Submit a picture of the Nominee either as part of the electronic form, or in the mail to:

• Print out this release form that gives us permission to publish the Nominee's picture, sign it, and mail it in along with the picture to the address above.
